A root operation broke the contract of a store (`scoped` mode, outside production by default).
Thrown by a participant from OperationParticipant::end() after it already cleaned up, and rethrown by OperationScope::end() only after every participant ended and the frame was closed, and never when the operation itself failed with an exception (that primary exception wins). Carries the operation name and the violation codes only: no request, payload or entity.
